When considering your appointment, please bear in mind that a typical comprehensive exam is often 45-60 minutes.  You may request appointment times on the hour beginning at 9:00 a.m. until noon and 2:00 p.m. until 4:00, Monday, Wednesday and Friday; on Tuesdays and Thursdays appointments are scheduled until 6:00 p.m.

We will contact you the same or next business day to complete the scheduling process.

While verbal communication is often more expedient, if you prefer to be contacted via email, be sure to provide that information above.


If you would like to complete your forms in advance of your appoint-ment, simply go to our Forms page to print out the appropriate History Form.
