When visiting Family Vision Care, we want to make sure that we have the most up-to-date information possible to best meet your eye health needs. With this goal in mind, we ask that you complete the History Form appropriate to your needs and bring it with you on the day of your exam.

If you have had an exam previously at our office, please select to from the following History Form choices:

(1) Welcome Back Adult (ages 16 and up),
(2) Welcome Back School Aged Patient (ages 6 -15), or
(3) Welcome Back Infant, Toddler, Preschool-Aged Patient

If this will be you first time visiting Family Vision Care, please select from the following History Forms:

(1) New Adult (ages 16 and up)
(2) New School-Aged Patient (ages 6-15)
(3) New Infant, Toddler or Preschool-Aged Patient

Before age 1, ask about the InfantSEE program.
Family Vision Care is proud to be a part of this program that enables
infants to enjoy a vision check at no charge. If your child is between the ages of 6 and 12 months, call our office to schedule this important check-up.

Once you've scheduled your infant's appointment, feel free to print and complete the InfantSee Confidential History Assessment form:
